I use a HIPAA secure documenting forum called Milknotes. This allows the patient to take time and fill out questionaires and review my policies before the home visit. I can communicate with patients through secure messaging through the site as well. The patient portal will be sent to you via email once an appointment is scheduled. Lastly, a personal care plan will be made for baby and parents.

Under 15 no fee
$25 for 15-20 minutes
$35 for 20-40 minutes
$50 for 40-60 minutes
As of now I am not in network with BCBS Federal and Medicaid. I will offer 50% off standard rates. You will be provided with a Superbill to submit to your insurance for potential reimbursement.
Cancellation fees – Appointments cancelled with less than 24 hours notice are subject to a $50 fee. This fee is not covered by insurance.
