I use a HIPAA secure documenting forum called Milknotes. This allows the patient to take time and fill out questionnaires and review my policies before the home visit. I can communicate with patients through secure messaging through the site as well. The patient portal will be sent to you via email once an appointment is scheduled. Lastly, a personal care plan will be made for baby and parents.
Cigna Patients
If you have Cigna commercial plans, Cigna affiliated third-party administrators (known as TPAs), and any corporate clients that say that is affiliated with click the link below to obtain insurance approval before scheduling your consultation.
I am in network with– Aetna, Amerihealth, or their subsidiaries, BCBS(not listed above), Capital Blue, IBX, Highmark, Keystone Health Plan East, and United, SO YOU DO NOT need to verify insurance first – you can skip right to booking!
Home Visit Travel Fees
Please note that a travel fee will apply for home visits:
- Under 15 miles, no fee
- $25 for 15-20 minutes from Chalfont, PA
- $35 for 20-40 minutes from Chalfont, PA
- $50 for 40-60 minutes from Chalfont, PA
Self Pay Clients
If you do not have coverage for lactation visits through your insurance, you may still book an appointment with us directly as a self-pay client. For self-pay clients, we will provide you with a superbill and necessary documentation to submit for out-of-network reimbursement from your insurance company. Self-pay clients should skip to our calendar and reserve an appointment time that best fits your schedule.
As of now I am not in network with BCBS Federal and Medicaid. I will offer 50% off standard rates. You will be provided with a Superbill to submit to your insurance for potential reimbursement.
Cancellation fees – Appointments cancelled with less than 24 hours notice are subject to a $50 fee. This fee is not covered by insurance.
